Required Disability Documentation

To specify the disability documentation requirements that will qualify (i.e., support current and essential needs) Yavapai College students for reasonable and appropriate accommodations.

General Eligibility Requirements

Each applicant with a disability must meet Yavapai College admissions requirements, or be enrolled as an Yavapai College student, and must provide Disability Resource (DR) with required documentation verifying the nature and extent of the disability prior to receiving any accommodation. The disability services office coordinator is responsible for evaluating documentation and determining accommodation eligibility.

Eligibility and Documentation

A student with a disability is eligible to receive reasonable accommodation if the documented disability is a physical or mental impairment that substantially limits one or more of the major life activities. Major life activities include: caring for oneself, performing manual tasks, walking, seeing, hearing, speaking, breathing, learning, and working. To begin the process to receive accommodation the student must:

Contact the Disability Resources Office and provide documentation of medical, psychological or other appropriate diagnostic evaluations, as requested, to document the nature and extent of the stated disability.

Special Consideration

The requirement for documentation may be considered at the discretion by the college's disability services coordinator. Such consideration is in the best interest of the student and will neither undermine the integrity of any college offering nor violate any mandate under state or federal law. All situations shall be considered on an individual, case-by-case basis.

Reasonable accommodation is required for students with known disabilities. YC will make every attempt to provide "preferred" accommodations. However, "reasonable and appropriate" accommodation(s) may be determined to meet sufficient accessibility needs.

Procedure for Requesting Accommodations

The student schedules an intake interview with Disability Resources (DR). The disability resources coordinator conducting the interview and the student discuss the condition and its functional limitations. Services are explained and the student’s rights and responsibilities are reviewed.

Students are required to submit documentation of their disability

Documentation is reviewed and evaluated by the DR coordinator. Students will be notified once their documentation is reviewed as to whether or not the documentation has met the established guidelines and verifies the existence of a functional limitation relevant to the academic environment.

The process of determining reasonable accommodations is collaborative among the student, the disability coordinator, the professional providing the diagnosis and the course instructor or program director if necessary. An Instructor Notification Form will be issued to the student once reasonable accommodations are determined.
